Torque Converter Shudder Diagnosis and Repair Guide

The lockup clutches lining came off from within the torque converter,I was told it may be caused by low oil pressure but we took the car back under warranty. Perhaps someone can explain to me exactly what torque converter (TC) shudder is. Can it happen when the lock up works fine?
